For most cats on a typical municipal water supply, tap water is perfectly safe. The main reason owners switch to filtered water isn't safety — it's palatability. Chlorine and other treatment chemicals can affect taste and smell in ways that put some cats off drinking, and a basic carbon filter (like the ones built into most cat fountains) removes a lot of that.
If you're on well water, or you have any specific concern about your local water quality, that's worth checking against your municipal water report or a vet's advice rather than guessing — filtering is about taste for most households, not a substitute for knowing your water source is safe.
As a general rule: a full water change and basin rinse every 3–4 days, with a proper deep clean (basin, pump, and impeller) every 1–2 weeks to keep biofilm and mineral buildup from forming — that buildup is exactly the kind of thing that makes water taste "off" to a cat and defeats the purpose of the fountain.
Filter cartridges are separate from the water itself and typically need replacing every 2–4 weeks depending on the model and how many cats share it — check your specific fountain's manual, since this varies more by brand than the water-change schedule does.
Some cats do find them more interesting — a bobbing ice cube can turn the bowl into something worth investigating, and it works especially well when the cube is made from a bit of low-sodium broth rather than plain water (see the hydration tips above). Usually it comes down to one of a handful of physical, fixable causes rather than the cat being "stubborn."
